The Village of Yellow Springs is opening up the opportunity of free Biometric Health Screenings to not only include Village Government Employees but eligible workers within the Village of Yellow Springs. These screenings help to provide an overall review of health and wellness, as well as identify any areas that may need additional support. 

You will be able to earn a $75 gift card by:
1) taking the health assessment online
2) scheduling a biometric screening
3) choosing your reward after your screening has been uploaded

• Individuals that participated in the past click on “Returning User”. (PLEASE NOTE: If someone participated in the past they must use their prior username, they will get an error if they attempt to create a second account.) If they forgot or aren’t sure they can call us at 855-206-1306 and request their username/password.

QUICK TIPS

Setting up a new account? 
If you are making a new account, please know that it will take a few days from creating your account and taking the assessment and then scheduling your biometrics appointment. I created my account and took the online assessment and then had to wait from Friday to Monday to be able to set my appointment, check your emails for information on your account being processed. I want to point this out, because if you wait until closer to the date, you might miss out. If you have any questions beyond the information I’m providing here, please feel free to reach out. 

Have an existing account? 
If you already have an account from a past year or with a previous employer you need to use the same information to receive your reward. They recommend that you use the name on your driver’s license when creating an account, if you used Timmy last time and try using Timothy this year it won’t match and there can be a delay in receiving your rewards. 

Account is now created!
After you have created an account, you are able to download the app, but you cannot create an account from the app. Create the account FIRST. Appointments can be made from the ActiveHealth app after the account is created and the information is received.
